Dental caries an ancient disease afflicted humans since centuries, the pattern of occurrence is not stable and varies globally. It is on the higher end in few countries and manifests itself in the lower end in others. It shows a different picture within the country and amongst the countries and the factors influencing this includes, political, social, economic and commercial changes that have occurred in these countries other than dental care delivery. The study of change in distribution of dental caries, helps in planning, policy making and also to prevent and control of this pandemic disease. The present paper is an attempt to review the global scenario of dental caries amongst 12 year old, and reasons for such a trend.
